Latest Updates to Martin's 'K-S IV'

Updated all three of my GUI/additions for K-S IV (Martin's Karplus synth) to add:

- All three Vibrato params have been made adjustable and given panel controls.
- Pitch controls can toggle to Semitone or Octave.
- Adjustable Pitch Bend range.
- Output Pan control.
- Added Spogg's Steve Howe preset, but changed the name to Steev How? to avoid lawsuits.

No change to Martin's 'engine'.
